{"id":30294,"date":"2024-04-15T11:22:52","date_gmt":"2024-04-15T09:22:52","guid":{"rendered":"https:\/\/hubi-vinciane.be\/projects\/balanced-diet\/prendre-soin-des-enfants-malnutris\/"},"modified":"2024-12-10T15:09:06","modified_gmt":"2024-12-10T14:09:06","slug":"caring-for-malnourished-children","status":"publish","type":"page","link":"https:\/\/hubi-vinciane.be\/en\/projects\/balanced-diet\/caring-for-malnourished-children\/","title":{"rendered":"Caring for malnourished children"},"content":{"rendered":"\n<p class=\"has-orange-color has-text-color has-link-color has-medium-font-size wp-elements-1 wp-block-paragraph\"><strong>The Project<\/strong><\/p>\n\n\n\n<p class=\"wp-block-paragraph\">With the &#8220;<strong>Care for Malnourished Children<\/strong>&#8221; project, we aim to combat severe and moderate malnutrition in children under 5.<\/p>\n\n\n\n<p class=\"has-orange-color has-text-color has-link-color has-medium-font-size wp-elements-2 wp-block-paragraph\"><strong>Objectives<\/strong><\/p>\n\n\n\n<p class=\"wp-block-paragraph\">We want to significantly reduce the number of deaths as well as prevent relapses and exacerbations.<\/p>\n\n\n\n<p class=\"has-orange-color has-text-color has-link-color has-medium-font-size wp-elements-3 wp-block-paragraph\"><strong>Project Description<\/strong><\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Thanks to our ongoing screening campaigns, we have identified three types of malnutrition, each with appropriate treatment:<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li><strong>Severe acute malnutrition without complications <\/strong><br><strong>(MAS or Malnutrition Aigu\u00eb s\u00e9v\u00e8re Sans complications)<br><\/strong>Children suffering from that type of malnutrition are referred to outpatient nutrition centres in local health centres.There, they receive treatment based on ready-made therapeutic food and, if necessary, medication according to the national care protocol.<br><\/li>\n\n\n\n<li><strong>Severe acute malnutrition with complications <\/strong><br><strong>(SAMAC or Malnutrition Aigu\u00eb s\u00e9v\u00e8re Avec Complications)<\/strong><br>Children with that type of malnutrition are admitted to therapeutic feeding centres in hospitals. According to the national care protocol, they are cared for there for an average of two weeks. The Foundation covers all treatment costs and provides follow-up care during and after treatment.<br><\/li>\n\n\n\n<li><strong>Moderate acute malnutrition <\/strong><br><strong>(MAM or Malnutrition Aigu\u00eb Mod\u00e9r\u00e9e).<br><\/strong>Children with moderate acute malnutrition are treated in their villages, in the <em>Foyers d&#8217;Apprentissage et la R\u00e9cup\u00e9ration Nutritionnelle (FARN)<\/em>. They receive enriched local food there. During cooking workshops organized by the FARN, mothers learn how to make their children&#8217;s diet more balanced and rich with locally available foods.<\/li>\n<\/ul>\n\n\n\n<p class=\"wp-block-paragraph\">After the intensive treatment period, our social workers continue to follow up with the children at home for two to three months. During these home visits, they advise family members about healthy eating habits and good hygiene practices. They also measure and weigh the patients each time.<\/p>\n\n\n\n<p class=\"has-orange-color has-text-color has-link-color has-medium-font-size wp-elements-4 wp-block-paragraph\"><strong>Results and Evolution<\/strong><\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Between 2019 and the end of 2023, we diagnosed more than 2,000 children with <strong>acute malnutrition with and without complications<\/strong>. Almost all those children (95%) received treatment at ambulatory nutrition centres.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Thanks to the rapid and preventive interventions among moderately acutely malnourished children, we managed to reduce the percentage of severely malnourished children from 3.15% in 2019 to 0.35% in 2023.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Nevertheless, there are still children who die as a result of malnutrition. After all, parents &#8211; for various reasons &#8211; do not always permit their children to receive treatment. The various screening campaigns for severe acute malnutrition that we have been running since 2019 have significantly reduced the number of malnourished children.<\/p>\n<!-- \/wp:paragraph -->\n\n<!-- wp:paragraph -->\n<p>By 2023, annual screening has been carried out in over 6,000 families in the 47 villages of N'Dali, where we started the project in 2015, and in 16 villages of Tchaourou, where we began in 2022.<\/p>\n<!-- \/wp:paragraph -->\n\n<!-- wp:paragraph -->\n<p><strong><em>Among these families, our 13 social workers screened at least 12,100 children aged between 6 and 59 months, 50.07% girls.<\/em><\/strong><\/p>\n<!-- \/wp:paragraph -->\n\n<!-- wp:paragraph -->\n<p>In Tchaourou, the malnutrition rate fell from 8.83% at the start of the project to 2.9% in 2023, <strong>a drop of 5.93 points<\/strong> in less than two years. This 5.93% drop is partly due to our <strong>Mother-Child Couple Nutrition and Health Promotion (PNSCME) project<\/strong>, through which we promote the importance of balanced, healthy meals in local communities.<\/p>\n<!-- \/wp:paragraph -->\n\n<!-- wp:paragraph -->\n<p>We also refer severely malnourished children to hospitals or medical centres for intensive treatment. Children suffering from moderate malnutrition are monitored at home. Indeed, the majority of the Beninese population has low purchasing power and lives in isolated areas with very poor access roads. All these factors limit availability of quality food and care. The high household size due to polygamy, multiple births, and low use of family planning methods, influences the nutritional status of children. In addition, there are several food taboos and prohibitions in Benin. In the north of Benin, the population struggles to practice exclusive breastfeeding. Some ethnic groups consider colostrum unfit for consumption. Others refuse to give meat, fish, or eggs to their children because of the risk of predisposing them to theft. In still other ethnic groups, fathers are given priority in the sharing of meals, especially with regard to eating meat.\u201d<\/p>\n<!-- \/wp:paragraph -->\n\n<!-- wp:paragraph {\"textColor\":\"orange\"} -->\n<p class=\"has-orange-color has-text-color\"><strong><em>How is severe acute malnutrition managed?<\/em><\/strong><\/p>\n<!-- \/wp:paragraph -->\n\n<!-- wp:paragraph -->\n<p><strong>The hospital social worker<\/strong>: \u201cAcute malnutrition is treated at various levels of the health pyramid depending on the form (moderate, severe without complication or severe with complication) and following an elaborate protocol. Severe acute malnutrition with complications is treated in therapeutic nutrition centres, which are mostly located in the paediatric wards of regional and departmental hospitals, for an average of 14 days of hospitalisation.\u201d<\/p>\n<!-- \/wp:paragraph -->\n\n<!-- wp:paragraph {\"textColor\":\"orange\"} -->\n<p class=\"has-orange-color has-text-color\"><strong><em>How is the symbiosis of the actions of the different actors involved in the care process achieved?<\/em><\/strong><\/p>\n<!-- \/wp:paragraph -->\n\n<!-- wp:paragraph -->\n<p><strong>Eric<\/strong>: \u201cIn the municipalities where the Foundation Hubi &amp; Vinciane operates, a dynamic team consisting of a paediatrician, nurses, care assistants, social workers and nutritionists work together on the various components of the treatment to ensure that the children are taken care of. Medication treatment is provided by the paediatrician, nurses, and care assistants. It aims to treat all diseases associated with malnutrition. Nutritional treatment and psychosocial treatment are provided by the assistant under the supervision of the paediatrician. I register the children and take anthropometric and clinical parameters to establish a file for each patient. I educate the mothers or carers on the procedures to follow during treatment. I prepare the therapeutic milks (F75 or F100), elaborate the daily food ration for each child, the distribution of meals and the follow-up of the administration of the meals to the children.\u201d<\/p>\n<!-- \/wp:paragraph -->\n\n<!-- wp:paragraph {\"textColor\":\"orange\"} -->\n<p class=\"has-orange-color has-text-color\"><strong><em>What is the role of the nutritionist in the management of a malnourished child?<\/em><\/strong><\/p>\n<!-- \/wp:paragraph -->\n\n<!-- wp:paragraph -->\n<p><strong>Eric<\/strong>: \u201cI act as a liaison between the hospital and the community. I screen cases in the community and refer them to the hospital's paediatric department. I am the direct link with the social worker for a good follow-up of malnourished children discharged from the hospital. I organise home visits to the parents of malnourished children. During these visits, I educate the parents on healthy and balanced nutrition and many other topics to prevent a probable relapse. I also provide cooking demonstrations to show balanced recipes adapted to ensure the good nutritional status of children recovered from malnutrition.\u201d<\/p>\n<!-- \/wp:paragraph -->\n\n<!-- wp:paragraph {\"textColor\":\"orange\"} -->\n<p class=\"has-orange-color has-text-color\"><strong><em>What does the mother or carer do with the child?<\/em><\/strong><\/p>\n<!-- \/wp:paragraph -->\n\n<!-- wp:image {\"align\":\"right\",\"id\":19203,\"width\":400,\"height\":263,\"sizeSlug\":\"large\",\"linkDestination\":\"none\"} -->\n<div class=\"wp-block-image\"><figure class=\"alignright size-large is-resized\"><img src=\"https:\/\/hubi-vinciane.be\/wp-content\/uploads\/2021\/02\/2021_Q1_acute-Ondervoeding_03_800.jpg\" alt=\"De Stichting helpt kinderen\" class=\"wp-image-19203\" width=\"400\" height=\"263\"\/><\/figure><\/div>\n<!-- \/wp:image -->\n\n<!-- wp:paragraph -->\n<p><strong>The social worker<\/strong>: \u201cDuring this 14-day period, the mother or guardian of the child receives training on balanced nutrition and the preparation of various fortified porridges. She is also made aware of food and body hygiene. This training enables her to better care for the child at home after the hospital treatment. It should be noted that it is not always easy to refer a malnourished child detected in the community.\u201d<\/p>\n<!-- \/wp:paragraph -->\n\n<!-- wp:paragraph {\"textColor\":\"orange\"} -->\n<p class=\"has-orange-color has-text-color\"><strong><em>What are the difficulties encountered?<\/em><\/strong><\/p>\n<!-- \/wp:paragraph -->\n\n<!-- wp:paragraph -->\n<p>The most affected areas are often inaccessible. Mothers of children find it difficult to leave home to come and do the 14 days of treatment at the risk of losing their husbands or remaining children. We also have a very low standard of living which does not allow the mother to easily last the 14 days because the child is taken care of but not the mother. We also note the low level of involvement of the husbands. It is at the root of more than 45% of the deaths of children aged 0-59 months. In Benin, 32% of the under-five child population suffers from stunted growth. Within this same population, 5% of children are emaciated. Pregnant and lactating women are another target for malnutrition due to deficiency because nearly 58% of women aged 15 to 49 are anaemic.<\/p>\n<!-- \/wp:paragraph -->\n\n<!-- wp:paragraph -->\n<p>The complexity of the immediate and underlying causes of malnutrition is one of the reasons that make it difficult to eradicate this condition because it affects several sectors at the same time. It is, therefore, necessary to establish a synergy of action between the sectors concerned.<\/p>\n<!-- \/wp:paragraph -->\n\n<!-- wp:paragraph -->\n<p>For some years now, the <strong>Hubi and Vinciane Foundation<\/strong> has been implementing a synergy of action between the education, health and agriculture sectors for an effective fight against malnutrition due to deficiency in children under five years of age, pregnant women and breastfeeding women.<\/p>\n<!-- \/wp:paragraph -->\n\n<!-- wp:paragraph -->\n<p>Through <strong>the agriculture sector<\/strong>, several projects have been implemented to ensure food security for the population at the grassroots level and the empowerment of women. Several screening campaigns for acute malnutrition have been carried out in the intervention villages. Malnourished children detected during these campaigns are cared for free of charge at different levels of the health pyramid according to their form of acute malnutrition. Nutritional education and cooking demonstration sessions are regularly organised within the community to help them prepare their meals with the available food.
